Abstract: The application of positive end-expiratory pressure (PEEP) is one of the key components of the ventilatory management of acute lung injury. In addition to the central hemodynamic effects the perfusion and oxygenation of the splanchnic region is affected. The intrathoracic blood volume (ITBV) is a useful indicator for the volume therapy. ITBV-guided volume therapy can maintain the perfusion. The combination with low-dosed dobutamin has no significant benefit for the perfusion and oxygenation of the liver and the small intestine. Under dobutamin-infusion the heart-frequence increased significantly.
