Abstract: To evaluate the long-term clinical and socioeconomic outcome of structured transition care in adolescents with inflammatory bowel disease (IBD). We compared 24 patients with and 11 patients without structured transition care within a follow-up period of 4 years. The prevalence of intestinal complications and the cumulative radiation exposure, as well as the costs for admissions and surgeries were reduced in transition patients compared to patients without structured transition.

Hintergrund der Arbeit war die Evaluation der klinischen und sozioökonomischen Auswirkungen auf den Langzeitbehandlungsverlauf von Jugendlichen mit einer chronisch entzündlichen Darmerkrankung (CED). Insgesamt 24 Patienten mit Teilnahme an einem strukturierten Transitionsprogramm wurden mit 11 Patienten mit rein formaler Überweisung an den weiterbehandelnden Arzt über einen Beobachtungszeitraum von 4 Jahren verglichen. Nach Transition zeigten sich weniger intestinale Komplikationen, eine geringe Strahlenbelastung und tendenziell niedrigere Ausgaben für Operationen und Krankenhausaufenthalte. [German]
