Abstract: PSP and ALS are neurodegenerative diseases whose clinical presentations can overlap in cases of concurrent frontotemporal dementia and upper motor neuron syndrome. PSP is a 4R tauopathy, while ALS is a TDP-43 proteinopathy; this distinction is therapeutically important but difficult to make without specific biomarkers. The aim of this study is the ante mortem differentiation of PSP and ALS using morphometric measurements of the midsagittal midbrain to predict their underlying histopathology. The results show that the midsagittal midbrain area does not enable this differentiation.
